  • This form is only to be completed by families with one family member living in the domicile that has an underlying Medical condition. Please complete the following questions in order for your student's attendance to be considered to be exempt for the 2020-2021 school year. This will be reviewed by administration and you will receive an email or phone call back verifying the final decision on your student's attendance expectations.
    ***If your student(s) is approved for a Medical Exemption from attending school daily, your student is deemed ineligible to participate or attend any extracurricular activities sponsored by the school.

    Responses are due no later than 8 a.m. on Thursday, August 6th.

  • The novel coronavirus, COVID-19, has been declared a worldwide pandemic and is extremely contagious. As a
    result, in order to resume school activities, social distancing and other essential safety measures at the Catholic
    School named above ("School") have been established. The School has put in place reasonable preventative measures and standards of behavior to reduce the spread of COVID-19 at School and School activities. Even with implementation of such safety protocols, the School cannot guarantee that I or my child(ren) will not become infected with COVID-19. Attendance at School and/or participation in School activities could increase my risk and/or my child(ren)'s risk of contracting COVID-19. On behalf of myself and my child(ren), I expressly assume this risk.

  • Authorization form for school personnel to administer medication.

  • The annual health update needs to be completed in March at re-enrollment and in August at the beginning of the school year.

  • Included with the application file is a household letter that explains how to complete the application. Please return your application to your building secretary. All applications will be approved/denied in the order they are received and you will receive a letter in the mail letting you know if your child(ren) qualify.

  • If the physical is done after May 1, it is good for the next school year. The NSAA requires schools to use their forms. The following students are REQUIRED to have physicals on file in the office of their respective building before participating in any sport:

    4th, 5th, & 6th graders participating in PAL volleyball and/or basketball
    All 7-12th graders participating in any sport
    All 7-12th graders participating in cheer and/or dance
    ALL 7th graders must have a physical on file in the MS/HS office prior to attendance regardless of participation in activities
    The NSAA Pre-Participation Physical Form MUST BE USED.

  • Parents of all students grades 7-12 participating in any of the following activities MUST complete the NSAA Student & Parent Consent Form prior to participation in those activities.

    Football, Volleyball, Cross Country, Girls Golf, Softball, Play Production(One-Act Play), Music(Instrumental and/or Vocal)
    Basketball, Wrestling, Swimming, Speech
    Track, Boys Golf, Baseball, Tennis, Soccer
